Physician – Post-Acute Care (Riverdale - Bronx, NY) We are hiring a full-time, quality-focused physician to lead clinical care across skilled nursing facilities in the Bronx, NY. What You Will Do Patient Care: Deliver comprehensive primary and post-acute care to a diverse patient community (Monday–Friday rounding). Collaboration: Partner with interdisciplinary facility teams, patients, and families to manage care transitions and treatment plans. Quality Improvement: Utilize data-driven population health tools and participate in value-based shared savings programs to optimize outcomes. What We Offer Compensation: Estimated $223,000–$263,000 annually, with uncapped productivity upside and shared-savings incentives.
