Weekend Hospice Registered Nurse (RN) – Direct Hire Location: Gap, Pennsylvania Schedule: Full-Time Weekends | Two Shift Options Travel: Local Travel Required Education Requirement: Associate Degree or Higher Overview Our client is seeking a compassionate Weekend Hospice Registered Nurse to join their extraordinary care team in Gap, PA . The Weekend Hospice RN will manage and deliver individualized patient care plans, support patients in Gap and surrounding areas, and consistently meet critical deadlines for charting and documentation. Key Responsibilities Provide management and delivery of individualized hospice patient care plans Support patients in Gap, PA and surrounding areas Conduct comprehensive assessments and ongoing evaluations of patient needs Consistently meet critical deadlines for charting and documentation Collaborate with interdisciplinary hospice team members to ensure coordinated care Educate patients and families on care plans, symptom management, and available resources Self-manage time, schedule, and daily patient visits Maintain compliance with all regulatory, clinical, and organizational standards Schedule & Shift Details Full-time weekend schedule with two available shift options: Friday / Saturday / Sunday — 8:00 AM to 8:00 PM Saturday / Sunday / Monday — 8:00 AM to 8:00 PM Required Qualifications Associate Degree or higher Valid Registered Nurse (RN) license in Pennsylvania (PA) 2+ years of Hospice experience Strong ability to self-manage time, schedule, and documentation Compassionate approach to end-of-life care Excellent communication and patient advocacy skills Additional Requirements Valid Driver's License Reliable Mode of Transportation
