Travel Physical Therapy Assistant (PTA) – Home Health Sacramento, California Mercy Home Health Contract Duration: Mar 31, 2026 – Jun 30, 2026 (13 Weeks) Estimated Weekly Pay: $1,877.84 Schedule: Day Shift | 8-Hour Days | 40 Hours/Week Position Overview Trinity Healthcare Talent Services is seeking a Travel Physical Therapy Assistant (PTA) for a Home Health setting in Sacramento, CA. This role involves providing in-home therapy services , assisting patients in improving mobility, independence, and functional outcomes in their home environment. Facility Information Setting: Home Health / Post-Acute Care Coverage Area: Sacramento region Charting System: HomeCare HomeBase (HCHB) Contract Type: LOA Coverage (Urgent Need) Clinical Responsibilities Patient Care Provide therapy services under PT supervision Assist with: Strength and mobility training Gait and balance improvement Functional independence Work with patients recovering from: Surgery Chronic illness Mobility limitations Home Health Duties Travel to patient homes Evaluate home environment for safety Educate patients & caregivers on: Exercises Equipment use Home safety Clinical Skills Assist with: ROM assessments Therapeutic exercises Prosthetic training Functional mobility training Documentation Maintain detailed records using HCHB system Complete: Progress notes Clinical summaries OASIS-related documentation Collaboration Coordinate with: Physicians PTs Nurses & home health aides Participate in case conferences Scheduling & Work Details 8-hour day shifts Full-time (40 hrs/week) Mileage reimbursement (GSA guidelines) Timesheets must be approved by supervisors Required Qualifications Active California PTA License Minimum 2 years experience Home Health experience (REQUIRED) OASIS experience (REQUIRED) BLS Certification Valid driver's license + auto insurance NPI Number Preferred Qualifications HomeCare HomeBase (HCHB) experience Strong communication & documentation skillsAbout the company
