Travel Physical Therapist (PT) – Outpatient Lakeport, California Contract Duration: May 4, 2026 – Aug 3, 2026 (13 Weeks) Estimated Weekly Pay: $2,616.89 Schedule: Monday–Friday | 8:00 AM – 4:00 PM (5x8s) Position Overview Trinity Healthcare Talent Services is seeking a Travel Physical Therapist (PT) for an Outpatient setting at Sutter Health Lakeside Hospital . This role focuses on delivering individualized therapy programs to patients in an outpatient environment, helping restore mobility, function, and quality of life. Facility Information Facility: Sutter Health Lakeside Hospital Location: Lakeport, CA Setting: Outpatient Schedule: 5x8s (No call, no floating) Contract Length: 13 Weeks Key Responsibilities Perform patient evaluations and assessments Develop and implement customized treatment plans (POC) Provide therapy for: Orthopedic conditions Post-surgical rehab Mobility and functional impairments Deliver: Therapeutic exercises Manual therapy Gait and mobility training Monitor patient progress and adjust care plans Educate patients on recovery and home programs Maintain accurate documentation Serve as a clinical resource when needed Clinical Highlights Outpatient-focused role No call, no floating Structured weekday schedule Strong pay package for CA Required Qualifications Active California PT License (REQUIRED at submission) BLS Certification (AHA or Red Cross) Minimum 2 years of PT experience Degree from accredited PT program Preferred Qualifications Outpatient experience Epic EMR experience Strong ortho rehab skills Compliance Notes 2 references (within last year) required Sutter cover sheet mandatory 7-year work history verification required Must complete labs within 3–5 days of signing RTO: Only 1 holiday allowed Local candidates accepted at lower rate (within 50 miles) Why Consider This Assignment High-paying outpatient role No call / no floating Clean weekday schedule Work with reputable Sutter Health system Great for outpatient-focused PTs Ideal Candidate Profile Strong outpatient PT experience Comfortable managing independent caseload Good documentation + communication skills
