Travel Physical Therapist (PT) Valley City, North Dakota Home Health 13-Week Assignment Day Shift | Monday–Friday Pay Package Estimated Weekly Pay: $2,400/week Position Overview Trinity Healthcare Talent Services is seeking a Travel Physical Therapist (PT) for a home health assignment in Valley City, ND . This role involves providing therapy services to patients in their homes across a 60-mile radius , treating a mix of post-surgical, fall recovery, and general home health cases. Key Responsibilities Conduct in-home patient evaluations and develop care plans Provide skilled PT treatment to home health and hospice patients Manage a caseload of 5–7 patients per day Document care using HomeCare HomeBase EMR Educate patients and families on mobility, safety, and recovery Coordinate care with interdisciplinary teams Requirements Active North Dakota PT License Minimum 1 year of Home Health experience REQUIRED BLS Certification (AHA) Valid Driver's License & Auto Insurance REQUIRED Strong time management and independent work skills Assignment Details Schedule: Monday–Friday | 8:00 AM – 5:00 PM 40 hours guaranteed No weekends or holidays Coverage area: ~60-mile radius No interview required (fast submission process) Important Notes Must live 50+ miles from facility Cannot have worked with CommonSpirit/CHI/Dignity within past 12 months Facility may cancel up to 4 shifts per contract RTO must be submitted upfront
