We're seeking an experienced Primary Care, Internal Medicine, or Geriatric Physician to serve as a clinical leader on our collaborative care team. What Makes This Opportunity Unique? Rather than managing a demanding daily patient schedule, you'll focus on clinical leadership, collaboration, and improving patient outcomes. You'll oversee and support a team of 3–4 experienced Nurse Practitioners and Physician Assistants, providing clinical guidance and expertise to ensure exceptional care for patients with complex chronic conditions. Your role will include: Providing clinical oversight, consultation, and mentorship to APPs Reviewing complex cases and developing collaborative treatment plans Conducting limited patient visits when physician-level care is needed Participating in interdisciplinary care coordination and quality initiatives Utilizing innovative technology, including AI-assisted documentation and remote patient monitoring This is a primarily office-based position with limited patient visits, allowing you to positively influence the care of a much larger patient population.
