About the position

Position: Integrated Care Nurse Practitioner (Hybrid) Schedule: Monday–Friday | Flexible hours Hybrid Role: Guaranteed 1 day per week remote Travel: Statewide travel ~80% of the time (4 days/week) Occasional overnight travel (company paid) Compensation: Base salary + 15% quarterly bonus Perks: Mileage reimbursement + company cell phone Sign-On Bonus Available Overview We are seeking Nurse Practitioners or Physician Assistants who enjoy variety and flexibility. This role supports patients with chronic and complex conditions, including kidney disease, through a mix of statewide in-home visits and telehealth . Youll work as part of an integrated care team alongside a Registered Nurse and Social Worker. Key Responsibilities Conduct comprehensive assessments and annual wellness visits (home & virtual) Develop and manage individualized treatment plans Prescribe medications and order diagnostics Coordinate care with physicians, specialists, and interdisciplinary team members Educate patients and families on disease management and preventive care Complete timely, accurate clinical documentation Requirements Active Nurse Practitioner or Physician Assistant license and board certification Active DEA license 2+ years of direct patient care experience Ability to practice independently Willingness to travel statewide (overnight travel paid) Valid drivers license and reliable transportation Benefits Medical, dental, vision, and life insurance 401(k) with 100% vested employer match from day one Paid vacation and holidays FSA & HSA options Pet insurance Mileage & cell phone reimbursement Sign-on and quarterly bonuses
