Family Nurse Practitioner (Primary Care) - OR

About the position

Position Summary We are seeking a dedicated Family Nurse Practitioner (FNP) to provide comprehensive primary care and health promotion services across the lifespan. Working under the direction of licensed medical providers, this role delivers patient-centered care focused on preventive health, chronic disease management, and health education. The FNP will collaborate closely with a multidisciplinary care team while adhering to established clinical standards, protocols, and organizational values. Key Responsibilities Provide health promotion and preventive services, including school physicals, immunizations, well-child and well-woman clinics, adult primary care, blood pressure screenings, nutritional counseling, family planning counseling, and prenatal care as applicable Conduct individual and group health education activities Administer or prescribe appropriate treatments and medications within scope of practice Perform or order laboratory tests, X-rays, and other diagnostic procedures Obtain, perform, and document comprehensive patient histories and physical examinations Evaluate and treat minor illnesses and injuries Monitor and manage chronic diseases in collaboration with a supervising physician Maintain strict patient confidentiality and comply with HIPAA requirements Complete accurate and timely clinical documentation in accordance with clinic protocols Refer patients and obtain consultations for services not provided within the clinic Promote health through counseling on diet, hygiene, preventive care, and lifestyle modifications Perform medical and minor surgical procedures within scope of competency Provide additional services as agreed upon with clinical leadership Education & Experience Completion of a Master of Science in Nursing (MSN) Graduation from an accredited Family Nurse Practitioner program (required) Qualifications Active and unrestricted Oregon license as a Family Nurse Practitioner (required) Current DEA registration and CPR certification (required) Current Basic Life Support (BLS) certification Strong communication skills with the ability to interact effectively and tactfully with patients, physicians, and staff Working knowledge of regulations, policies, and clinical standards applicable to community health centers, particularly FQHC environments Ability to work independently while contributing effectively within a collaborative, team-based care model Demonstrated commitment to patient confidentiality and ethical practice Cultural sensitivity and experience serving diverse populations Flexibility to work occasional evenings and weekends as needed Professional, courteous, and patient-centered demeanor Strong teamwork and collaboration skills
