Palliative Care Nurse Practitioner (Hybrid)

Job Title: Palliative Care Nurse Practitioner Location: Field-based within assigned market Salary: $135,000 to $150,000 + company bonus program Travel: Local travel required for in-home patient visits Job Overview: We are seeking a Palliative Care Nurse Practitioner to support a growing interdisciplinary care team providing in-home care for patients with complex chronic conditions. This role focuses on delivering compassionate, patient-centered palliative care to individuals living with advanced illnesses such as CKD, ESRD, diabetes, COPD, and CHF. The Nurse Practitioner works closely with the Market Physician Executive and a multidisciplinary care team to support symptom management, advance care planning, and improved quality of life for patients and their families. This role emphasizes population health management, shared decision-making, and coordinated care delivery for medically complex patients. Key Responsibilities: Identify patients who qualify for palliative care services in collaboration with clinical leadership Develop individualized care plans based on evidence-based interventions Provide palliative care through primarily in-home visits with virtual care as needed Assess and treat symptoms affecting quality of life, including pain, depression, anxiety, insomnia, and other conditions Collaborate with pharmacists and clinicians to manage medication plans for complex patients Educate patients and families on disease progression, treatment options, and palliative care principles Facilitate advanced care planning conversations and document patient wishes using appropriate state forms Support families in navigating complex medical decisions and understanding prognosis Coordinate care with interdisciplinary team members, including social workers, chaplains, and other clinicians Assist with hospice transition when appropriate Participate in clinical leadership meetings and contribute to program performance goals Provide education to care team members on palliative care practices and symptom management Required Qualifications: Master's degree in Nursing Board certification as a Nurse Practitioner Active state licensure with willingness to obtain additional licenses if required Minimum of 2 years of experience in a palliative care setting Reliable transportation and the ability to travel within the assigned region Preferred Qualifications: ACHPN or CHPN certification Experience developing evidence-based care plans for complex patients Strong communication skills for sensitive conversations regarding prognosis and end-of-life care Experience documenting complex cases within electronic health record systems Benefits: Competitive salary with bonus program Comprehensive medical, dental, vision, and life insurance Paid time off 401(k) with employer matching Opportunity to work in a fast-growing organization focused on transforming chronic disease care
